I/we the parents of guardian named above, authorize the ministry leaders, volunteers or staff of Penticton First Baptist Church to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessments, treatment or procedures for the participant named above.
I/we, named above, unertake and agree to indemnify and hold blameless the ministry leaders, volunteers and staff of Penticton First Baptist Church, its pastors and Board members from and against any loss, damage or injury suffered by the particapant as result of being part of the activities of the Beyond Youth group, as well as of any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when particpanting in events with Beyond Youth group and Penticton First Baptist Church.
Are we able to take photos of your child participating in activities? These photos will only be shared within Beyond Youth group or for Penticton First Baptist Church presentations and Social Media platforms?
